Alpha Hydroxy Chemical Peels

Alpha hydroxy acids (AHA) are a group of acids that are often found in fruit, sour milk, sugar and other products processed through bio-fermentation. AHA’s have an illustrious history in the cosmetic industry, as they are believed to be the first chemical peels used by doctors over the past decades. Topical formulations incorporate AHA’s frequently used and prescribed by doctors and are also present in a wide variety of the skin care products available now such as cleansers, moisturizers and skin fresheners.

Types of AHA’s typically used in chemical peels for skin are:

  • Glycolic acid – derived from sugar canes
  • Lactic acid – derived from milk
  • Malic acid – derived from apples and pears

  • Tartaric acid – derived from grapes

Of these, glycolic acid is the most popular and can be combined with these other AHAs in chemical peel formulations.

Alpha hydroxy acids work mainly as an exfoliant. They cause the cells of the skin (epidermis) to become “unglued”, degrading the lipids that hold the most outer layer of dead skin cells to slough off, making room for re-growth of younger-looking, new skin. In addition AHAs stimulate the production of collagen and elastin, which provides greater strength and protection of these skin cells. The result is skin that is smooth in appearance and texture.

Alphahydroxy acids (AHAs) are the mildest of the chemical peel formulas and produce light skin peels. These types of peels can provide smoother, brighter-looking skin for people who can’t spare the time to recover from the more intense modified Jessner, Jessner and TCA chemical peels. Candidates for AHA chemical peels are those with skin impairments such as:

TCA Chemical Peel – 15% & 20%

Tricholoracetic acid (TCA) is a chemical solution. It is a strong acid made by chlorinating acetic acid – and is widely used as a chemical skin peel. This procedure aims to improve the skin’s appearance by removing the outer layer of damaged skin to reveal the underlying layer, which is newer and smoother in texture.

TCA chemical peels are more effective than alpha hydroxy acid (AHA) chemical peels (i.e. glycolic acid) in removing superficial skin, but does not penetrate as deeply as carbolic acid skin peels (i.e. phenol). It is generally referred to as a “medium depth” chemical peel. The main targets of a TCA chemical peel procedure are those individuals with:

  • Fine surface wrinkles
  • Pigment discoloration (blemishes and blotches)
  • Age (liver) spots
  • Superficial acne scarring
  • Dark circles under the eyes

The advantage of the technique of tricholoracetic (TCA) acid application is that the degree and depth of penetration within the skin can be controlled accurately according to specific requirements. The concentration of TCA in the chemical peel is an important factor that alters the depth of the peel. Concentration can vary from 15 to 50%, with the higher concentrations leading to a deeper peel.

Medium peels are generally performed as office procedures on an out-patient basis, and require approximately 45-90 minutes. TCA peels will cause a heating sensation that will last about two to ten minutes after which it will self-neutralize.

Within a few days, your superficial layers of skin will turn darker, become stiff, and leathery. The skin will then crack, flake, and peel with flaking usually complete in four to seven days. Once your old skin has sloughed, your new skin will be bright and flushed.

Typically, you will be able to return to your normal activities immediately, and most people can comfortably return to public within a week. Your flushed appearance will fade slowly over several weeks.

Unlike deep peels, medium peels may be performed safely on people with olive and light brown skin. However, people with dark brown skin are at a higher risk of discoloration.

Medium chemical peels will last six months to two years. If you use medical-grade skin care products regularly after your peel, you will maximize the duration of improvement. Most women who choose medium chemical peels as the mainstay of their facial rejuvenation have a repeat peel every six to twelve months. TCA chemical peels provide an intermediate level of improvement at an intermediate cost and recovery period. They are ideal for individuals who want more than a superficial peel, but cannot afford the expense or recovery time of a deeper treatment.

How it works

What to expect

The Obagi Blue Peel procedure takes less than an hour in your medical practitioner’s office and does not typically require sedation. Your skin professional will apply the Blue Peel typically to your face and neck. A mild burning sensation will occur that generally lasts only a few minutes. Then you may experience a bluish tint to your skin that usually washes off in 12 to 24 hours. Your skin professional will advise you on how many peels are needed to give you the desired result.
Depending on the strength and depth following the procedure, you will experience light swelling. Within two to three days, your skin will begin to peel. This peeling continues over the next several days. By the seventh to tenth day, your skin should be healed.
Once your skin has completely recovered and is finished peeling, you will notice a visible improvement in your complexion. Your skin’s appearance should continue to improve for the next four to six weeks, becoming firmer, clearer, smoother, healthier and more youthful looking.

Before the Peel

Pre-treatment creams are applied to the face at night for several weeks prior to the peel. By exfoliating the skin and reducing pigmentation themselves, they improve the results seen from chemical peeling. They may also reduce the time needed for healing. The creams usually include one or more of the following:

  • Tretinoin (Retin-A, high-potency retinols, Differens, Renova, Tazarac)
  • Alpha-hydroxy acid e.g. glycolic or lactic acid
  • Broad-spectrum SPF 30 (UVA/UVB) sunblock must be used during the day

The Peel

Superficial chemical peels are a minor procedure and no special arrangements are needed. Moderate-depth chemical peels are more aggressive, and clients must be prepared for the procedure. First, the face is thoroughly cleansed with alcohol to remove surface oil. Next, the peeling agent is applied for several minutes. It may sting and become warm (hot) – how badly and for how long depends on the chemical agent used and its concentration. A fan is available to assist in comfort. The peel is then neutralized, and the heating sensation diminishes. Individual treatments may include peels with several agents on the same occasion with the aim of improving results and reducing risks.


Complications are uncommon if the health professional performing the peel is properly trained.

  • There is a rare possibility of an allergic reaction. You should discuss thoroughly with the Registered Nurse any such reactions and understand them. If you are concerned about allergic reactions, you should request a patch test.
  • This treatment is a noninvasive, light exfoliation consisting of singly, or a combination of Salicylic Acid, Lactic Acid, Glycolic Acid, Resorcinol, Trichloracetic Acid, Retinoic Acid and Enzymes.
  • The use of the above ingredients stimulates the skin to generate new skin cells. It does not replace deep chemical peels, laser resurfacing or plastic surgery.
  • There may be some degree of discomfort during application. You will notice a warm sensation and the skin may tingle, sting, pin pricking, heat (burn) or tightness. Immediately after the chemical exfoliation treatment, your face may appear frosted or red, and by day two (2), the skin may darken in color, feel tighter, and be more sensitive. Days two (2) through seven (7), the skin may exfoliate. You must not to pick or peel skin. Pulling or picking skin may lead to infection, hyperpigmentation and or surface scars.
  • You may experience some breaking out after a treatment.
  • You MUST avoid direct sun exposure.
  • Chemical Exfoliation treatments may lighten hyperpigmented skin, reduce acne breakouts or diminish fine lines. There is NO GUARANTEED result. In some cases, there could even be an increase of uneven color from this procedure.
  • Please inform the registered nurse if you have not been using Accutane, Differin®, Azelex®, Finacea™, Tazorac®, Renova®, tretinoin or any other prescribed medication(s) for the past two weeks.
  • Please be aware that if you are prone to cold sores (Herpes Simplex), this treatment could stimulate cold sore eruptions. You should begin a prophylactic treatment with prescription anti-virals about 3 days prior to the chemical exfoliation treatment and continue for about 7 days after the treatment.
  • Please inform the registered nurse if you are aspirin sensitive. Aspirin sensitivity could cause a reaction most specifically to chemical exfoliants containing salicylic acid.
  • To achieve maximum results, you may need several treatments and use home care products.
  • This treatment is a cosmetic treatment and no medical claims are expressed or implied.
  • Comedomes (blocked pores) or acne may result from the peel itself or from thick moisturizers used afterwards.
  • Scarring may result from infection or picking the scabs and can be permanent.
  • Blotchy pigmentation is most likely in those with darker skin or who had a pigmentation problem before the peel; keep out of the sun and use hydroquinone.

Looking after the skin after the peel

The first days after your chemical peel (or until your skin is completely healed), follow this program using a medical-grade skin care program:
  • Wash your face with the cleanser every morning and every night.
  • Apply moisturizer 3 to 4 times a day.
  • For itching use a 0.5% hydrocortisone cream 3 to 4 times a day.
  • If oozing occurs, soak gauze pads in a 3% hydrogen peroxide solution and dab the area as needed.
To help achieve optimal results and healing:
  • DO NOT PICK, RUB OR FORCE OFF SKIN. You will be tempted, but it is imperative that you keep the skin on your face as long as possible. Prematurely removing the skin will lengthen the healing process, potentate scarring and infection, and sting when products are applied.
  • Avoid sun exposure and the use of sun screens until healing has occurred—wear protective clothing, a hat and sunglasses.
  • Avoid strenuous exercise and sweating.
  • Do not allow water or shampoo to run onto the treated areas while bathing or showering.
  • WE RECOMMEND YOU DO NOT WEAR MAKE-UP WHILE THE SKIN IS HEALING. If you feel you must use make-up, we recommend using only a medical-grade mineral make-up such as gloMinerals. This can be applied a few hours after the procedure.
